In consideration of participating in an activity at Conway Regional Health and Fitness Center (Center), I acknowledge the risks associated with exercise and physical activity. I am aware of possibility of accident or other physical injury during physical activity, but I affirm that my general health is good and that I am not adversely affected by physical activity. By checking the box below, I agree that all use of Center's facilities, programs, and services shall be undertaken at my sole risk and that Center shall not be liable for any injuries, accidents, or death occurring to me, including those resulting from the Center's negligence, arising either directly or indirectly, out of my participation, or use of, the Center's facilities, programs and services. I, for myself and on behalf of my executors, administrators, heirs, and assigns, do hereby expressly release, discharge, waive, relinquish, and covenant on to sue the Center, its affiliates, officers, directors, agents, or employees for all such claims, demands, injuries, damages, or cause of action, with respect to the use of Center's facilities, programs, and services.
